ROBIN L. WILSON, COMMISSIONER. The claimant has filed a Motion to Stay Appeal Time Limitation from the June 13, 1995 Findings of Facts and Dismissal of the Commissioner acting for the First District. This motion was construed as a notice of appeal in accordance with the Chairman’s letter dated July 11, 1995. To date, the claimant has not filed documents in support of her appeal, including Reasons of Appeal or a brief of the issues. It is the appellant’s responsibility to provide this board with an adequate record for review once an appeal has been filed. See Sargent v. Rybczyk Plumbing, 13 Conn. Workers’ Comp. Rev. Op. 128, 130, 1660 CRB-6-93-3 (January 31, 1995). Therefore, we must dismiss her appeal for failure to prosecute with proper diligence pursuant to Practice Book § 4055. See Esquillin v. Pinto Lavado Enterprises, 13 Conn. Workers’ Comp. Rev. Op. 148, 1914 CRB-2-93-12 (Feb. 1, 1995); Burke v. Abacus Transfer & Storage, 13 Conn. Workers’ Comp. Rev. Op. 19, 1782 CRB-3-93-7 (Nov. 3, 1994).
Commissioners Nancy A. Brouillet and Michael S. Miles concur.
